Fill out your paperwork
Before your first visit, we do require a few dental forms to be completed. For your convenience, we’ve made these forms available on our website so that you can print and complete them before your first visit. Please print and complete the forms above to bring to your new patient appointment. We suggest that you arrive at your first appointment 5-10 minutes early so that we can process your paperwork and answer any questions you may have. Also, remember to bring your dental insurance card with you (if you have one). Doing so will help our staff determine the type of care you need and any specific accommodations we should make.
Share any previous experiences or preferences
If this is not your “first-first” visit to the dentist, you probably have a story to tell, and we want to hear it! Let our staff know about any previous experiences you’ve had: the good, the bad and the ugly.
